Before your first session, please print these documents, read them carefully, and fill them out. These documents include a sheet explaining how my office works and a second copy of that same sheet. I ask that your read and sign the page telling youhow my office works and give me that signed copy for my records. The second copy of that page is for your records. The otherdocument includes all the information that I need in order to properly keep up with the accounting for your therapy. I will also need to make a copy of your insurance card so please bring that with you to our first session.

Some insurance companies do require pre-authorization for therapy. The best way to find out if your insurance company requires pre-authorization is to call the number on the back of your card and ask them. If they require pre-authorization, they’ll give you an authorization number. Please bring that number with you when you come to our first session.
